Hospice Physician: Works in conjunction with the patients Interdisciplinary Group (IDG) , for a patient center approach towards end of life care.
Registered Nurse: Manages the entire team caring for the patient. Manages the patients medical care and is highly skilled in assessing and managing pain and discomforts, while providing educational training to families and caregivers regarding patient care.
Certified Nurse Aide: Assist the patient with activities of daily living, such as bathing, grooming, dressing, etc.
Social Worker: In hospice the social worker plats a vital role in the care of the patient and their families. The social worker addresses financial issues, and helps obtain any governmental support, if the patient qualifies. The social worker also helps with end of life preparations such as funeral and mortuary arrangements.
Chaplain: Works with the patients clergy and is available to provide spiritual guidance and support for the patients and their families.
Hospice Volunteer: Trained volunteers are available to spend time with the patient, and offer companionship and provide relief for the family.
